Three Rivers Medical Center and Lawrence County Health Department are the lead agencies for the Access to Healthcare and Preventive Services priority issue:

Goals will be accomplished on a weekly, monthly, and annual basis continuing throughout the CHIP cycle. Workgroups will meet at least quarterly to review progress toward meeting goals. Workgroups will report progress to the Health Action Team on a quarterly basis. This plan is a living document and goals will be updated as needed.

Priority Issue #1: Access to Healthcare and Preventive Services

Goal #1: Increase the percentage of the population receiving preventive screening services.

Objectives / Date Completed

  • Develop and disseminate strategic marketing monthly campaign to increase community awareness of available preventive screenings beginning by January 2024.
  • Develop and/or use existing educational materials that are easy to understand and distribute to partners for dissemination by January 2024.
  • Host a minimun of 2 health fairs/screening events per year throughout the county beginning in 2024.

Goal #2: Reduce the percentage of the population living with chronic diseases.

Objectives / Date Completed

  • Develop and disseminate strategic marketing monthly campaign on making healthy lifestyle choices to reduce your risk of chronic diseases including healthy eating, physical activity, avoiding alcohol, avoiding tobacco and secondhand smoke, regular sleep, and oral health by July 2024.
  • Develop easy to understand educational materials and/or use existing on the above choices for partner dissemination by July 2024.
