Lawrence County Health Department is the lead agency for the Communication priority issue.
Goals will be accomplished on a weekly, monthly, and annual basis continuing throughout the CHIP cycle. Workgroups will meet at least quarterly to review progress toward meeting goals. Workgroups will report progress to the Health Action Team on a quarterly basis. This plan is a living document and goals will be updated as needed.
Priority Issue #4: Communication
Goal #1: Implement a collaborative communication plan to promote resources and provide education.
- Present Community Health Improvement Plan to local agencies/organizations to create awareness and solicit involvement and support by December 2023.
- Promote Health Action Team Facebook page and increase traffic by 25% by December 2025.
- Create a Health Action Tea website that provides education and resources on priority issues of obesity, healthy eating, physical activity, mental health, access to care, and preventative services by June 2024.
- Create a Speaker’s Bureau of Health Action Team members to engage in a minimum of 6 speaking engagements each year beginning January 2024 with at least 2 engagements addressing policy change.
- Provide monthly education and information through news outlets, billboards, social media, utility bills, bank statements, church bulletins, newsletters, grocery bag stuffers, ect., by June 2024.
